The very best beauty secrets somehow seem to make their way out into the open during Bravo’s Watch What Happens Live With Andy Cohen and Monday’s after-show segment was no exception.

As Real Housewives alum Heather Dubrow dished—after being prompted by a caller who asked her opinion on “preventative Botox”—she’s been getting injections for 22 years.

“It depends, person to person,” she said. “For me, personally, I started at 27.”

As Heather explained, that’s the age she met now-husband, plastic surgeon Terry Dubrow, MD, who told her, “If you just put a tiny little bit there, you’ll never get that line.”

“And I trusted him and 22 years later, I’m line-free. By the way, I’m going to be 50 in January and no surgery. I’m fighting it, non-surgically.”

While there may be no denying the power of prevention, the math deserves a double-take: Botox Cosmetic wasn’t approved for cosmetic use until 2002, which means the Dubrows were using it for about six years pre-FDA OK.

But Beverly Hills facial plastic surgeon (and Dr. Dubrow’s co-host on E!’s Botched) Paul Nassif, MD says Botox had, and still has, a long-standing history of FDA approval for therapeutic uses—such as providing patients with temporary relief from both strabismus and blepharospasm (spasms of the eyelid muscle)—before the cosmetic “go” came.

“While receiving Botox for therapeutic reasons, patients displayed outstanding cosmetic improvement and began to lose their frown lines and brow wrinkles. This is what initially sparked the “off-label” use of softening wrinkles by plastic surgeons and dermatologists.”

Plus, Dr. Nassif points out, now preventative Botox is more popular than ever, particularly amongst millennials

“It’s partially due to the fact that this generation is highly researched and familiarized with the aging process, and knowledgeable when it comes to prolonging the youthful appeal of their face, neck and body,” he says. “In my med spa, we use non-surgical procedures, such as Botox, as preventative measure within the 20–29 age range almost daily. By using the correct dosage, there are no disadvantages or side effects, aside from the occasional temporary swelling at the injection site.”
