Psoriasis doesn’t always stay where it’s easy to talk about. When it shows up on the genitals, it can affect intimacy and relationships, and treating it is tricky because higher-potency steroid creams generally aren’t recommended for routine use on such sensitive areas.

As reported by Dermatology Times, new analyses presented at the 2026 Fall Clinical Dermatology Conference in Las Vegas found that ZORYVE (roflumilast), available as a steroid-free cream and foam, reduced patient-reported sexual difficulties and relationship problems linked to psoriasis compared with the same cream or foam base without roflumilast, including in people with genital involvement.

Ahead, a closer look at the findings and what they could mean for people living with genital psoriasis.

What the research shows

The analyses drew on three phase 3 trials. DERMIS-1 and DERMIS-2 tested ZORYVE cream 0.3 percent in people with plaque psoriasis, while ARRECTOR tested ZORYVE foam 0.3 percent in people with scalp and body psoriasis. Roughly one in six participants had psoriasis in the genital area at the start (15.6 percent in the cream trials and 17.4 percent in the foam trial).

Participants rated how much their psoriasis caused sexual difficulties and problems with partners, close friends or relatives. After eight weeks, both measures favored ZORYVE over placebo, both overall and among people with genital psoriasis.

How it worked in sensitive areas

In the cream trials, 39.9 percent of people using ZORYVE reached treatment success on a doctor-rated scale at week 8, compared with 6.5 percent on placebo. Among those with psoriasis in the genital area, the numbers were 39.3 percent versus 5.4 percent. With the foam, 45.1 percent of people with genital psoriasis reached success, compared with 5.7 percent on placebo.

Itch relief stood out too. Among people with genital psoriasis, 71 percent using the cream saw a meaningful drop in itch, compared with 16.7 percent on placebo. For the foam, it was 63.6 percent versus 37.9 percent. These results reflect overall psoriasis severity and itch, rather than symptoms in the genital area specifically.

Quality of life followed suit. Among people with genital psoriasis who qualified for this analysis, 64.4 percent of people on the cream and 81.8 percent on the foam reported a meaningful improvement, compared with 34.6 and 33.3 percent on placebo, respectively.

Why a steroid-free option matters

Using topical corticosteroids for extended periods can cause side effects, including skin thinning, so lower-potency options are generally preferred for sensitive areas like the genitals. ZORYVE works differently. It’s a PDE4 inhibitor, a type of medication that helps reduce inflammation, and it’s formulated without ethanol, propylene glycol or fragrance.

Both formulas were generally well tolerated in the trials, including among people with genital psoriasis. Fewer than 1 percent of people reported a hot, tingling or stinging feeling with definite discomfort 10 to 15 minutes after the first application.

What this means for you

The groups with genital psoriasis were small, these analyses covered only eight weeks and most participants were white, so there’s more to learn. Still, the study authors suggest that dermatologists consider bringing up sexual function with patients who have genital psoriasis.

You don’t have to wait to be asked. If you’re not sure whether it’s psoriasis or another rash, or it’s getting in the way of your relationships, bring it up at your next appointment. Treatment is about more than clearer skin. It’s about feeling comfortable in your own body, and that’s a big part of sexual wellness, too.
