Johnny Franco, MD, FACS, is a board-certified plastic surgeon and founder of Austin Plastic Surgeon in Austin and San Antonio, Texas, who earned his medical degree from the University of Texas Medical Branch at Galveston, completed his plastic surgery residency as chief resident at Saint Louis University, and pursued advanced microsurgery and fat-transfer fellowships in Taiwan and Belgium. Widely recognized as an innovator and national leader in aesthetic body contouring and the current weight-loss and wellness wave, he has helped pioneer the integration of GLP-1–driven transformation, wellness, and regenerative techniques into modern plastic surgery practice. What are you most proud of about your practice?
What I’m most proud of is how we’ve been able to integrate and foster so many thought leaders under one roof. From our surgeons to our injectors, this isn’t built around one person. It is a full team of national speakers, trainers, and educators who are out there every week teaching, learning, and truly pushing the field of plastic surgery and aesthetics forward. We’re constantly cross-pollinating ideas on a variety of topics whether it’s weight-loss and body contouring, preservation breast techniques, regenerative fillers like Alloclae, or advanced medspa treatments—and that energy shows up in the results we deliver for patients. Because of these incredible providers, I genuinely believe our practice is seen both locally and nationally as a hub for innovation, a place where new concepts are created, refined, and shared with the aesthetic community.

Tell us about your doctor/patient relationship dynamic.
The physician–patient dynamic here at Austin Plastic Surgeon is a little different—in the best way. We do a huge amount of education through social media and digital content so that by the time patients walk through our doors, they already have an intimate understanding of their procedure and what the journey will look like from start to finish. We’ve worked really hard to take the “black box” out of aesthetics and make the entire process transparent, approachable, and real. My goal is that patients feel like they already know us and already know the plan, so that our time together in the consultation room can be deeper, more thoughtful, and truly customized. The vibe is fun, upbeat, and very interactive, but at the core, that is a very serious commitment to helping each patient safely and confidently reach their true aesthetic goals.

Using up to five examples, share the most interesting things about your practice or work that sets you apart.
One of the biggest things that sets our practice apart is how deeply we’ve leaned into the weight-loss revolution in aesthetics—I lived the GLP-1 journey myself, lost the weight, and then built out an entire program to help patients not just shrink down, but actually rebuild shape, skin quality, and confidence in a thoughtful, long-term way. We also don’t believe aesthetics should be just “skin deep,” so every treatment plan is built from the inside out. This involves changing the mindset about wellness, weight loss and aesthetics as it is not just about how someone is going to look and feel . It is not just about the next several years, but the next 20, 30, 40 years from now, especially as we see younger “Ozempic generation” patients with more mature issues like laxity and volume loss.
On the tech side, we’re very much AI-and-imaging forward in our practice. Using social media, education, 3D imaging, and emerging AI tools to demystify the process, dial in expectations, and make sure patients feel like they already know us and their plan before they ever step into the office, so our in-person time can be fun, honest, and incredibly productive.
We’ve also become a bit of a regenerative playground—from preservation breast techniques to new fat-based fillers like Alloclae and collagen-stimulating “collagen combos,” we’re constantly looking for safer, smarter ways to help patients age gracefully instead of just chasing quick fixes. And finally, we run a true open-door teaching/collaborative practice. We have surgeons and injectors visiting almost every week to share ideas and watch cases, which is one of the biggest compliments you can get from your colleagues and keeps us evolving, learning, and pushing the specialty forward together.

How do you manage patient expectations?
Managing patient expectations is one of the hardest and most important parts of what we do in aesthetics. We lean heavily on photos, imaging, and even our own social media so patients can see real examples and have a clear picture of what’s actually possible. I’m excited for how AI will eventually help make that even more personalized. Imagine a world where AI can search the internet to find similar bodies for and patients that have had the same procedure you are interested in having done. At the core, we try to under-promise and over-deliver, which is why every new surgical consult has at least two visits before their procedure. Those extra conversations go a long way in preventing miscommunication and making sure we’re all truly on the same page.

How do you show patients you care post-treatment?
For me, the real work doesn’t end when the procedure is over, that is actually where the relationship really starts. We put a huge emphasis on follow-up, whether that’s calling patients the night of surgery, checking in the next day, or just being truly accessible when they need us. I think you really see what a practice is made of when someone hits a bump in the road and things aren’t perfectly smooth; being there, staying calm, and helping them through that is how you show it’s not just about a single result, it’s about the whole journey. That long-haul mentality has turned a lot of our “patients” into incredibly loyal, almost extended-family members of the practice.

What one surgical advancement are you most excited about right now?
I really believe preservation breast surgery is going to change the entire world of aesthetic breast surgery, whether people are ready for it or not. Everything in medicine is moving toward less invasive procedures and faster recovery, and breasts should be no different. Preservation techniques have completely shifted how I think about breast surgery, regardless of what breast implant company you are using currently. The future is clearly moving toward fewer implants, biologic/regenerative options and more respect for a patient’s natural anatomy. What excites me is that we can use these concepts to maximize what you already have, support your own tissues, and still get the full, beautiful result you’re hoping for in your enhancement. Just in a smarter, more thoughtful, and more modern way.
Alloclae was hands down the most exciting product to hit the market in 2025, and I really believe it’s just the beginning. You’re going to see this whole category of treatments explode as more regenerative and biologic options roll out in 2026 and beyond. Products like Dermaclae from Tiger Aesthetics in mid 2026 and Lipoderma from Biocyte, which are already starting to make their mark in the aesthetic world. What makes this moment different is that we’re not just talking about hype; we’re finally getting real science and real studies behind these products, showing how we can use your own natural tissues, build scaffolds from your own biologics, and expand or support your fat in smarter, more predictable ways. The regenerative revolution is coming fast, and Alloclae is just the tip of the iceberg—I think the next few years in this space are going to be incredibly fun and game-changing for both providers and patients.

How are you helping to advance the field at large?
Advancing the world of aesthetics has really become a critical portion of our practice. The weight-loss revolution has probably been the biggest mindset shift I’ve seen in my career, nationally, internationally, and personally. I still remember when I got push bac for talking about weight loss in an aesthetic setting, and now it’s front and center. The wellness wave has shown all of us that we can do so much more than tweak someone’s reflection; we can actually help them feel better, move better, and live better. This changed how we treated patients and truly created something from the inside out.
On the innovation side, I’ve been blown away by what’s now possible. Procedures like Rib-X, where we can actually change the bony framework, are things I never imagined when I started training. And then you layer on preservation breast techniques, which we’ve helped expand from a single brand and concept into a whole menu of options. It is completely reshaping how we think about breast surgery in a more anatomic, tissue-respecting way. When you combine that with the weight-loss revolution, you get what I call the “Ozempic-ification of aesthetics” which has created really young patients showing up with mature issues like skin laxity and volume loss. We’re not just treating them for a few years we’re responsible for helping guide their face and body for the next 60–80 years, which means combination treatments and long-term planning are absolutely key.
A big part of pushing the field forward, to me, is not just doing the work in our own office, but sharing it. That means saying yes to national talks, sitting on panels, being honest about what works (and what doesn’t), and keeping an open-door policy so other surgeons and injectors can come visit, watch cases, and trade ideas. Those conversations, live surgeries, and real-world case discussions are where a lot of the magic happens. If we keep collaborating like that among the thought leaders across cities, and even countries, then I think the next decade in aesthetics is going to look completely different in the best possible way.

Why is giving back so important to you?
I think giving back to the specialty is incredibly important, because none of us got here on our own. Mentors, teachers, and colleagues poured into us, and this is how we keep plastic surgery at the forefront of aesthetics. Most people who go into plastic surgery had a lot of other options. They were the best and brightest in their classes and walked away from easier, more “predictable” paths for a life that’s demanding, messy, and incredibly rewarding in surgery and aesthetics. At the end of the day, we’re here to help people, and part of that is not just taking care of today’s patients, but investing in the next generation of surgeons so the specialty keeps evolving long after we’re out of the OR.
