What was your goal for the environment of your practice when first starting out?

We wanted to develop an ambiance of warmth and comfort a hominess to the feel, yet within a professional atmosphere. Most patients are taken aback by the decor, style and warmth. Patients say their anxiety of coming to see a surgeon is relieved by the warm atmosphere they feel when they walk in.

In what ways do you promote stringent safety standards?

My office includes a new 5,600-square-foot surgery center certified by the American Association for Accreditation of Ambulatory Surgery Facilities (AAAASF). Likewise, my nurses are certified in advanced cardiac life support, and only board-certified anesthesiologists are used in the operating room.

What steps do you take to augment patients' comfort?

Recovery-room nurses are bedside during patients' post-operative recovery, and a staff private-duty nurse makes home visits upon request.

After a mastectomy, what type of breast reconstruction gives a patient the most natural results?

Usually by using their own tissue, typically, a TRAM flap where the tissue from the tummy is transferred to the breast.

LIPOSUCTION

What is Body Contouring?

It’s shaping the body through either liposuction or skin removal surgery.

What is the best way to remove fat in the arms?

It depends on the skin tone. In patients with good skin, liposuction is best way. However, with poor skin tone, removing both skin and fat through surgery is the best way.

Do you need liposuction with a tummy tuck?

It depends. If a patient needs it, we perform liposuction with a tummy tuck or in adjacent areas like in the hip. It really depends on what the patient’s problems are.

What is a “good candidate” for liposuction?

Patients who have excess localized areas of fat with good overlying skin tone, and who are relatively close to their ideal body weight.

What is the best way for men to remove excess breast tissue?

The best way it can be done is with liposuction, where excess tissue and fat is removed through 1 to 2 small incisions around the breast. It’s often followed by some natural contracture of the skin which will help shape the breast after fat removal.

How do you get rid of “turkey neck?”

Turkey neck is treated when you do a facelift by contouring the muscles under the jawline and redraping and removing the excess skin. It can also be done as an isolated procedure known as a necklift. We make an incision confined to the area, behind as well as under the chin. The lax muscles are tightened, fat is removed and the excess skin is removed and redraped.

What is the difference between ultrasonic liposuction, laser-assisted liposuction (SmartLipo), tumescent liposuction, and power-assisted liposuction?

Tumescent refers to the fluid placed under the skin into the fat area. It has adrenaline in it to minimize bruising and bleeding; and also contains lidocaine to minimize pain. It serves to loosen the fat and is used as a medium into which ultrasonic liposuction can be safely performed. Ultrasonic liposuction is where the liposuction probe has an ultrasonic energy applied to the tip which helps emulsify the fat; particularly where the fat is bound in fibrous tissue, which allows easier extraction than with traditional techniques.

BREAST IMPLANTS

What type of implant should I choose?

The patient has a choice between saline and silicone. The advantage to silicone is that they provide a more natural feel in most patients. I think they’ve been shown to be safe, with a high degree of patient satisfaction; but neither saline nor silicone should be viewed as permanent. They last until they rupture. The rate for rupture is less with silicone than with saline. And at that point, they should be replaced.

Where is the best place for the incision when getting breast implants?

There needs to be a discussion between the physician and the patient. The patient often has a choice; but the most direct access is with an incision in the crease of the breast, which is ultimately hidden by the natural hang of the breast. But for patients who do not want the incision in that location, and if they have an areola that is the proper size, placing the incision there can be a nice alternative. There is a slightly higher risk of having diminished nipple sensation with going in through the areola.

BODY LIFT

What does a full body lift involve? Is it painful and is there a lot of scarring?

A total body lift is usually a combination of what we think of as a tummy tuck with an outer thigh lift and a buttock lift. It’s a circumferential removal of skin and fat and then a resuspension of the loose thigh and buttock tissues, as well as tightening of the abdominal musculature. It is fairly to moderately painful. The scaring extends like a belt around the lower abdomen or lower trunk.

CHEEK AUGMENTATION

Are chin and cheek implants inserted when performing a facelift?

Yes, they can be. I think we’re using chin implants more than cheek. Fat injections to the cheek areas are probably more popular.

How are cheek and chin implants inserted?

The options are either through incisions in the mouth or, with the chin, in the crease below the chin. Cheek implants can be put in an incision placed below the lower lashes.

What kind of implant is used for the chin or cheeks?

Usually solid silicone implants.

How is a breast lift performed?

It can be performed by removing excess skin, and in cases of minimum droopiness, and by placing an implant. When the breast is not able to be lifted, by making an incision around the areola will allow the breast to be shaped and lifted, but if there is significant droopiness to the breast, that periareolar incision will need to be combined with either a vertical incision under the breast or in combination of a vertical and horizontal anchor incision.

STRETCH MARK TREATMENTS

Will a tummy tuck get rid of stretch marks?

It will only remove the stretch marks within the area of skin that is removed. The ones that remain will still remain; although they will often appear better once that skin is stretched.

Are there other ways, besides surgical removal like in a tummy tuck, to remove stretch marks?

At the present time, there is no effective or reliable ways to improve stretch marks.

What is a mini tummy tuck? How does a patient know which they need?

It is a surgery where a small amount of skin is removed from the lower abs in conjunction with liposuction and muscle tightening in the area between the belly button and the pubis. Usually patients who have any amount of laxity in the muscle or skin above the belly button are candidates for a full abdominoplasty. If their skin tone is good and there is only laxity below the belly button than they are a candidate for a mini-abdominoplasty.

NOSE JOB

How do you fix someone’s nose, but make it still look natural?

To fix someone’s nose, typically most noses are done with open rhinoplasty with the incision inside and outside the nose, so we can lift skin back and work on those structures. Computer imaging helps determine what the patient’s expectations are for surgery, so they can feel comfortable

What is revision rhinoplasty?

It’s basically redoing a rhinoplasty that either has not met the patient’s expectations, or has not healed properly, or has suffered complications due to a previous rhinoplasty.

Is there a lot of swelling and bruising after a nose job?

There can be a moderate amount of swelling or bruising. If the nasal bone has to be addressed, then there is typically more swelling and bruising.

BREAST AUGMENTATION

How do I know what type of implant and size I need when getting breast augmentation?

Most patients come in with an idea of what size they would like to be. But, determining actual size needs to be done in conjunction with a surgeon who will determine the amount of skin and quality of the skin, as well as the dimensions of the chest, which then dictates a certain range of implant sizes that would naturally augment the breast. Three-dimensional computer imaging can help dramatically in showing the patient’s pre-existing asymmetries and anticipating results.

EYELIFT

How can I get rid of my droopy eyelids?

We can position the brow to where the patient thinks it’s in a good position. If there is still excess skin, than that skin needs to be recontoured to rejuvenate the upper eyelid.

What is the recovery like after a tummy tuck?

It’s similar to a c-section. During the first week, patients are mobile, and they are usually recovering at home without any amount of strenuous activity. They can resume some light exercise low impact cardio in 3 weeks; but they should wait 6 to 8 weeks before doing strenuous activity or weightlifting.

What is a mini tummy tuck? How does a patient know which they need?

FACELIFT

What is a liquid facelift?

It implies restoring volume and thereby lifting the lax skin of the face with off-the-shelf filler, such as Juvederm, Restylane, Sculpra or Radiesse.

What is a limited incision facelift?

It’s a facelift where the incision is limited as much as possible to the area around the earlobe, so that it minimizes the incision around the ear. I think one of the areas that are more difficult to conceal are incisions that go behind the ear and across and into the hairline. So, if we avoid those areas we have a more aesthetic result. Patients who don’t have significant excess skin excess may be candidates for a short incision. But, if they have more loose skin in the face and neck to be removed, it requires a longer incision to do that.

How do you get rid of “turkey neck?”

What is the recovery period for a facelift? Scarring?

Recovery varies in terms of the early recovery and late recovery. Most patients have swelling and bruising early on, which can last 7 to 14 days; and the majority will feel comfortable socially interacting in about 3 weeks.

What is the procedure for a facelift?

They vary depending on what technique is used. A traditional facelift has an incision that begins in the hair just above the ear and extends the natural contours behind and in front of the ear. It’s combined with a small incision under the chin. Through those incisions, the underlying muscle which carries the fat is suspended, contoured and tightened under the jawline; and then the excess skin is removed and draped off the newly contoured foundation.

BROWLIFT

How do you know if you need a brow/forehead lift?

Usually aging occurs throughout the entire face. If one area is lax, it is common for other areas to be lax. If the brow is in a very low position, and there is hooded skin in the upper eyelid, or if the brow is giving a stern or angry appearance than a brow lift can help rejuvenate someone’s appearance.

LASER LIPOSUCTION

What is Smartipo and when is it used?

I feel it has not really shown to have any significant benefits over traditional liposuction. In theory, it can help with skin tightening in areas where there is mild skin laxity. Although, it has really yet to be proven that it has any significant benefit in that regard.

What is the difference between ultrasonic liposuction, laser-assisted liposuction (SmartLipo), tumescent liposuction, and power-assisted liposuction?

What is a thigh lift?

There are two kinds: An inner and outer thigh lift, which is usually combined with a buttock lift. This surgery is designed to remove excess skin and fat from the thigh.

INJECTABLES AND FILLERS

How do I know what injectable fillers are right for me?

I think it depends on the areas being treated. Some are beneficial or better for areas with that need volume; and some are better for superficial wrinkle correction. Some people have an inherent ability to respond better to one filler or the other; but it mostly is dependent upon the areas being injected and the goal they want to achieve; whether it’s predominantly for line filling or volume filling.
