Neuromuscular Dentistry

Dr. Willardsen uses diagnostic technology to evaluate the cause rather than just symptomatology. Many smile-restoration patients are surprised to learn that their bites must first be corrected. “Crooked, broken and worn-out teeth may be a symptom of a misaligned jaw. Our job is to create the optimal bite before proceeding with cosmetic work.” According to Dr. Willardsen, some people never noticed their bites were “off.” “It’s like a pebble in your shoe. After a while, you learn to ignore it. There is no sense in cosmetically altering a smile when the foundation is flawed.” Twelve pain-less computer-generated diagnostic tests are administered for further accuracy. “We can track patterns, place the patches to measure stress on the chewing muscles and use little magnets between the lips and teeth to mark the correct trajectory. About my practice:

Does your office have its own on-site dental lab?

Dr. Willardsen: Yes. Because we have our own on-site dental lab, we are able to have more control of our results while creating beautiful smiles. Having our ceramists in house really makes a difference. Our patients really appreciate having the relationship with the ceramists as well as the doctor.

What is unique about your practice in terms of its ambience?

Dr. Willardsen: The atmosphere is commensurate with highly trained and qualified staff. It's part of the luxury experience discerning patients expect and receive.

How do you go about generating more genuine-looking smiles?

Dr. WIllardsen: My commitment to excellence through natural techniques gives me the insight on how to proceed. In my opinion, nothing is worse than a contrived look. My goal is for every patient to leave looking absolutely natural. Teeth are designed to complement facial type, complexion and overall expression. It's highly individualized.

What is the ultimate goal of your practice?

Dr. Willardsen: We want to provide long-lasting, beautiful, healthy results in an atmosphere that is commensurate with a quality experience. Aided by our highly trained staff, that’s part of the luxury experience discerning patients expect—and receive here. 

What vision do you both share?

Dr. Willardsen: We try to provide the best materials and the best techniques that we can to our patients. This practice has been busy and successful for many years and including a new partner is just part of its evolution—we felt that we could provide an even better service to patients with two dentists. 

In what ways is your approach all-encompassing?

Dr. Willardsen: We don't just stop at whitening teeth and replacing veneers; we provide patients with alternatives. In particular, we use cosme_canique (a massage-based manipulation of tissue) to firm, smooth and promote improved skin tone, thereby decreasing lines and increasing circulation. Couple that with facial training and certain exercises, and you can improve facial tone and create a smile lift.

What is your relationship with Dr. Willardsen like?

Dr. Cressman: This isn’t just a business relationship. It’s a personal relationship that makes us a stronger practice. I really respect what Dr. Willardsen has built here and the reason we joined practices is because I have the same passion for dentistry that he does and share his goal of providing the best dentistry services.

What have you done to advance your schooling?

Dr. Willardsen: I trained at the well-regarded Las Vegas Institute for Advanced Dental Studies, Arrowhead International and the Dr. Dick Barnes Group. I also studied at Occlusion Connections and am on its board of directors. Continuing education has made us aware of exciting techniques in cosmetic dentistry, which we incorporate at our practice. We also have a lecture hall so we can bring together industry leaders and teach others about the latest advances. 

How do you help spread the word about the importance of oral health?

Dr. Willardsen: I am a regular on local television, making frequent appearances where I help viewers select from among new dental techniques, including smile makeovers utilizing porcelain veneers.

TEETH WHITENING

What is the best way to get rid of discoloration of the teeth?

It depends on if they are stained. Bleaching will do a great job, but a lot of people that have done bleaching with no result, can use a no-prep veneer, where we don’t need to shave down teeth.

Do at-home white strips and whitening toothpastes work?

I think white strips work pretty well.  Whitening toothpastes removes surface stain, but don’t pull stain out of the tooth like a bleach would. I personally don’t think whitening toothpastes penetrate deep enough like peroxide does.

When whitening the teeth, are their foods you should avoid?

Avoid anything with a lot of color that would stain a white shirt. Avoid these types of food for the first week or so while whitening.  Whitening can open up the pores of the tooth, making it more susceptible to staining.

SMILE MAKEOVER

What is a smile makeover?

My vision of a smile makeover is taking anyone that is unhappy with their smile, and correcting it through orthodontics (straightening the teeth), porcelain veneers and/or bonding. In order to be successful, their smile needs to be functional as well. This means if the teeth are not in the right position, or the proportions are not right, than it cannot be a successful smile makeover.

How can you tell if my bite is in correct alignment?

Usually there are a lot of signs, such as the teeth not being in alignment, if there is spacing between the teeth or if they are crowded, if the teeth are chipped or worn down, if there is a lot of pain in the TMJ joint, or if they have headaches or neck pain.  When your teeth are apart, the lower jaw is in a relaxed position; but as your teeth come together, it puts force on the muscles, teeth and joints. If all is lined up, than that force is ok and all is in balance; but, if the teeth are not in line, you might hit on one side more than the other or chew on one side, or there could be spacing between teeth or flaring in the front teeth. We can use a computer to help u s find the exact lower jaw rest position; and we move the teeth based on that rest position.  It’s called “jaw tracking;” we follow the path of closure until the teeth touch. We want all the teeth to come together at the same time. 

What is the difference between bonding and veneers? What do they correct? When should each be used?

A lot of times they are interchanged. Veneers are porcelain coverings over the tooth that will close spaces, and fix cracked, worn-down, or chipped teeth. Bonding is done by hand in one appointment by the dentist. Veneers are done in a lab by a lab technician. It typically takes two appointments. In the past, bonding would change color and stain, but the material they use now is getting better.

How do you replace teeth that are missing?

The gold standard would be with implants, but there are other viable options such as bridges, where you would cap or veneer the two adjacent teeth and put a prosthetic tooth in the middle to fill the space.  Or we can use some sort of removable partial or denture as a temporary measure. Also, you can use a snap-on-smile to replace teeth temporarily.

How do you fix teeth with extensive decay?

There are two ways. I would use a biominetic restoration with advanced materials that flex and give and are placed using a very special technique; or, if it was completely necessary, I would put a crown on the tooth. The idea behind biominetic dentistry is that if you have a tooth with extensive decay, you would want to rebuild the tooth just like it was originally. A natural tooth will flex and give. If you re-build it with a composite or restoration, you want the middle portion of the tooth to be softer than the outside portion, just like a real tooth. We remove all the decay, clean it out and then we use a carbon mesh liner in the tooth and layer the composite in a certain way.  When the tooth is done, it has the same strength as the original one.  With a traditional inlay, or a metal filling, it is very rigid, so as the tooth flexes or gives, it creates a space between the tooth and the filling material. Then the tooth cracks from force and torque.  If it has elasticity in the restoration, it will last a long time. 

Can I make my gums pinker in color?

Yes. I’m not really familiar with it; but I’ve seen it done a few different ways.  One technique is to bleach the gums. Another technique is to scrape the top layer of tissue off with an abrasive material and it grows back pink. 

What is a “snap-on smile?”

It’s a plastic, fabricated smile that snaps over the existing teeth that can visually correct crowding, spaces, misshapen and worn-down teeth. It’s removable, and is usually used for special occasions or if someone has a missing tooth and they are waiting to replace them. It’s not very functional. It doesn’t chew well, and not something you would want long-term.

How are lasers used in dentistry?

Lasers are used more and more. Most dentists own a soft-tissue laser for shaping gum tissue, removing or helping heal cold sores or canker sores. A hard-tissue laser (which is more expensive) can cut teeth or hard tissue like bone, and can help fix a gummy smile. Certain lasers can also remove veneers safely. 

What is anti-aging dentistry?

Patients in their 20s come in with nice smiles, and patients in their 60s and 70s come in with broken and chipped teeth. So the idea behind anti-aging dentistry is how we can prevent this from happening.  It all has to do with the way the teeth come together and function.  If the bite is out of alignment, then teeth will wear out.  I advocate the anti-aging smile and how to keep it looking good.  Most dentists don’t recognize this, so patients keep wearing down their teeth.  But, you have to look at everything when you do cosmetic work. There are more adult patients in braces because that’s typically the problem. If teeth aren’t lined up properly than everything is off, and patients then start breaking and chipping their veneers. People are afraid of getting veneers because they hear others talk about them popping and breaking, but if everything is lined up, that won’t happen.

Can I make my smile better without having to get braces?

Yes.  It’s so subjective. When I meet with a patient, they have to break down what they don’t like:  Is it the front teeth, because chipped or is there space? Is it the whole smile or just a few teeth?  We can do veneers, bleaching, bonding, and simple contouring and shaping and if they don’t want traditional braces, or use Invisilign or a D & A (day-night appliance) appliance. With D &A there is a lot of tooth movement in 16 hours. It also can move bone. It is good for those with a narrow smile who want a wider one. It has a screw in it that you crank to widen the arch. Invisalign has to be used 21 hours-a-day, and it just torques the teeth. 

Who is the best candidate for Invisalign?

Anyone that has mild to moderate crowding. It’s also good for people who have spaces, but not good for rotating or pulling teeth down. 

How long do I need to do Invisalign to correct my smile?

Typically on average, it takes anywhere from six months to two years. 

How many trays do I need when using Invisalign?

On average 25 trays.  They are changed out every two to three weeks.
