Subtle Uplift

This patient is shown 60 days after one ePrime™ treatment and will continue to see improvement for another 30-60 days. The ePrime™ radio-frequency device tightens the skin and creates more elastic tissue in the area around the jowls and the wrinkles around the mouth. Though not a substitute for a facelift, the ePrime™ has been shown to provide a lifting effect.

What have you done to make beautiful skin more attainable?

My practice offers laser and aesthetic services and is home to my skin-care boutique, skinfo®, which offers more than 400 skin-care products, including its own eponymous line. It also hosts professional skin-care and makeup consultations, using skinfo® Mineral Makeup, Colorscience® and glominerals™ makeup and the novel VISIA® Complexion Analysis. I like skiing, tennis, weight training, playing with my son and watching professional sports.

I'm accessible, I listen and I care.

What treatment is best for Rosacea or redness in the skin?

I prefer Pulsed Dye Laser for pure rosacea, but some people don't respond to it. If they also have scattered brown spots or sun damage then I use IPL (Intense Pulsed Light).

What is the Pan G Lift?
It's a device that utilizes electricity to stimulate the muscles of the face. It's like exercising the facial muscles. There are small muscles that crisscross all over the face, and you can't isolate the muscles in the face like you can the other muscles in the body at home or at the gym. But, we can take poles and place them on each side of the muscles that lift the face, and make them contract. This device isolates all the lifting muscles in the face, and like isolating any muscle and repeatedly exercising it, the muscle responds by getting larger and more powerful over time. We do 20 treatments, over 10 weeks, gradually increasing the resistance each time. It's like increasing weight. This firming of the muscles creates volume and lifts the face. It looks very, very natural, and also helps maintain bone mass. It's like getting a personal trainer for the face. It's a big commitment, and only lasts as long as you keep it up. It also requires very skilled estheticians. You can only use it for the face because big muscles require so much energy to contract.
What is the best way to get rid of hyper-pigmented skin (age spots) caused by sun damage?

You can use IPL (Intense Pulsed Light) if you have a lot of brown spots. Or, if you have just a few, you can use a Q-Switched laser, which takes off individual spots very specifically. The Fraxel Dual laser has an added wavelength that can treat for spots, too. You can also use a combo of retinoids, lightening agents and chemical peels, if it's an all-over area. It does work for some people, but it depends on what level in the skin you can get to.

What can patients do to look younger?

I recommend Botox, fillers and tissue tightening. They're all nonsurgical and garner impressive results.

What is ePrime and what does it treat?

It's a new energy-based device that tightens tissue and ads volume in the face in a precise way by applying two rows of needles that send heat into the deeper part of the dermis where there are lots of cells that make new collagen. The higher temperatures stimulate those cells, achieving new elastic tissue and collagen. It's very consistent, unlike other tissue tightening devices (some are difficult to tolerate and depend on the thickness of the skin). With ePrime, we are firing the same place every time, so it's a huge stimulus that the body ignores. We are making fluffy new tissue to elevate the skin surface. It's a very natural looking correction, but in multiple studies, when compared to a facelift, it can only do a third of what a facelift can achieve.

After facial rejuvenation treatments, what's the best way to maintain the results?

I recommend the core four: Sunscreen, antioxidants, growth factors and retinols, or alphahydroxy acids (if they can't tolerate retinols). They should use sunscreen with five percent zinc oxide every single day. It all adds up. Antioxidants, when applied to the skin, help maintain skin health by soaking up free radicals before they damage the cell, and they are a cofactor to making collagen. You also need something to stimulate growth. There are two biologically relevant, complicated products that have growth factors in them that you can get from dermatologists: TNS serum by SkinMedica and Neocutis Bio-Restorative cream. And, you should use retinol or Retin-A to exfoliate dead skin cells as well as to stimulate the skin's natural collagen production. Or, if can't tolerate them, then AHAs. If you are super-sensitive, then you can use a Clarisonic face-cleansing brush. Anybody can use this regimen for skin health, if you are over 30. Before 30, use sunscreen.

It's very important if you have dark skin, to ask what type of laser to use and what kind the doctor has. Do your research; otherwise it's just not safe.

How can I get rid of stubborn cellulite?

Velashape and Thermage are both FDA-cleared to reduce cellulite, safely and effectively firming problem areas. In fact, Velashape is touted as the first FDA-cleared nonsurgical solution for circumferential reduction, slimming the body with absolutely no downtime. Likewise, Thermage uses radiofrequency vibrations to improve the appearance of cellulite noninvasively, smoothing, tightening and contouring for a leaner physique.

What is the best way to treat cellulite non-surgically?

I like two methods. ThermaCool uses specialized tips to reduce cellulite highly effectively. It is underutilized, probably because it's rather painful and expensive, but you only need one treatment. VelaShape is a device that combines broadband light and radiofrequency (energy based), as well as suction and movement (rolling) in order to reshape and push the fat down. It also tightens the skin, it kind of rearranges it. It's like a fat compactor, so it makes areas much smoother. VelaShape takes multiple treatments and it's painless, but you have to maintain it once a month, so it's time consuming. There's also a Brazilian technique, developed by Dr. Hedsel, where we cut the fibrous bands that form cellulite with a needle. With this procedure, however, we need to anesthetize and punch holes in the skin (so it's not totally non-invasive). There are some creams that also help with cellulite, but the effects are temporary.

I would like my face to look younger but am not ready for surgery. What nonsurgical alternatives are there?

Nonsurgical facial rejuvenation has generated unprecedented buzz in three areas: skin pigmentation, skin laxity and skin volume. CHEMICAL PEELS

Are there different types of chemical peels? How do I know which one is right for my skin?

With each peel, what changes is the chemical used and the strength of the chemical. In general, people usually use TCA peels (Trichloroacetic Acid), which are moderate peels; or glycolic and salicylic acid peels for something milder. A phenol peel is a deep peel, but more people are using a CO2 laser instead, which does the same thing with more precision. There are so many different peels on the market that are a combination and have multiple peeling agents in them. All a chemical peel can do is strip off the dead layer of surface skin and stimulate the metabolism of the cells underneath. This enables products to work more effectively.

What is a TRINITI laser treatment?

It's a combination of three different modalities: Intense Pulsed Light (IPL), radiofrequency, and diode laser. With the TRINITI laser, you need three treatments over three months and you get three treatments in one: Photorejuvenation (overall rejuvenation from sun damage), skin tightening and wrinkle reduction. It's an overall rejuvenation, for those who don't want needles or downtime. It's a great treatment.

What is an Erbium Laser Peel?

It's a brand of laser that is different because the erbium tends not to heat skin as much, and tends to be more ablative. One of the benefits of erbium laser resurfacing is minimal burning of surrounding tissue, so there is no heated skin left. With a CO2 laser, you have heated skin left. With the erbium laser you don't have as many problems with hyperpigmentation afterwards; but with the CO2, you have the advantage of tightening the skin more.

What is the best laser for skin resurfacing?

The CO2 fractional resurfacing laser; or the erbium fractional resurfacing laser. If you are a good doctor, you could get good results with whatever device you have. The device doesn't matter as much as the doctor does. You should have a qualified doctor perform the procedure to make sure it is done right and that it is effective because there is a lot pre- and post-op that goes along with it.

What is the best laser treatment for wrinkles?

I would use the CO2 resurfacing fractional laser.

How is a Fraxel laser different from other lasers?

Fraxel has a number of different lasers. Fraxel was the first to use fractionated technology with infrared light. It's much safer than what we had in the past, it was pretty revolutionary. The original was non-ablative (not a CO2 laser), which can be aggressive without much oozing. There were no open wounds, so there was no infection. There are not too many copies of the original Fraxel. Then they went on to design the Fraxel Repair CO2 device. There are a million copies of those.

Is there a way to tighten loose sagging skin that in non-surgical?

It depends on where there is sagging skin. We can use a skin tightener (such as ThermaCool and Titan), where the heat energy goes through the skin and is generated in the middle part of skin to make new collagen. ThermaCool and Titan tighten tissue with absolutely no down time, but it's not easy to predict who the best candidate for that is; it depends on the degree on sagging and where it is. The more laxity there is in the skin, the harder it is to treat with the least invasive tools. For moderate laxity, we would volumize with fillers under the eyes, in the cheeks and around mouth. We volumize the skin when the face loses fat, bone density and muscle mass, these all cause skin laxity, so, we can create tightening by using fillers. We are not technically tightening the skin, but pushing it out. A third way is by using an energy-based (radiofrequency) device, which puts needles under the skin to stimulate the skin to make new collagen and elastin, so we can get a more consistent result.
