Gone Today, Hair Tomorrow

For patients who are losing hair or are experiencing thinning hair, hair restoration procedures are at their peak, with a wide range of choices. “Hair loss isn’t just something men experience—women are also affected by thinning hair and hair loss,” says Dr. Tam. She offers a host of treatments to restore fullness and vitality for both men and women alike. “We offer a number of solutions—including topical treatments, oral medication, laser therapy, and even surgery.” Hair restoration surgery calls for donor hair to be taken from the back of the scalp and transferred to areas where the hair is thinning or lost. “Hair transplantation can garner long-lasting results that are undetectable, elevating a patient’s self-esteem and reducing self-consciousness. With a combination of different therapeutic options and customization of your treatment plan, we can actually improve areas of concern.”

Rapid Resurfacing

To diminish the signs of aging on this 55-year-old patient, Dr. Tam used an ablative skin resurfacing laser called ActiveFX. This fractional CO2 laser delivers thousands of small laser energy pulses to heat deep layers of tissue to treat skin irregularities and stimulate new collagen production. “This patient suffered from sun damage and wrinkles, which made her skin appear prematurely aged,” Dr. Tam says. “We used ActiveFX on her face, eyelids, neck and chest to give her a more even tone and smooth out fine lines.”

Philosophy

Dr. Alison Tam delivers deluxe dermatology procedures that focus on the use of multiple therapeutic modalities to achieve a higher level of skin health. “Our patients receive the benefit of greatly reducing the risk of future age spots, wrinkles, pre-cancerous lesions and premature aging. We aim to treat existing problems and prevent new symptoms from forming as well.” The practice also includes medical aestheticians and two plastic surgeons so that patients can appreciate a full range of skin care and cosmetic procedures all in one location for optimal results and convenience.

Rejuvenating Restoration

Suffering from male pattern hair loss for many years, this 49-year-old patient turned to Dr. Tam for help. “With hair restoration surgery, I was able to restore the hairline and fullness of his hair for a dramatically younger appearance overall,” Dr. Tam says.

About my practice:

What services do you offer?

We provide minimally invasive to highly invasive dermatologic, laser and surgical skin cancer and hair restoration procedures.

What upcoming dermatologic advances are you looking forward to?
I believe the future is in lasers. Laser therapy is always evolving. I spend a large part of my time educating my patients to help them choose a great approach for their skin. In the future, i believe that stem cell research may help us to better understand the process of aging.
What type of comprehensive skin-care program do you recommend?
I recommend coordinated care between myself, our plastic surgeons, and our team of medical aestheticians. Typically, this includes periodic treatments with the aestheticians and a home care regimen. My partners developed their own skin-care line to help maximize outcomes. Our practice also offers state- of-the-art lasers and surgical skin cancer and Hair Restoration treatments.
How do you calm anxious patients?
Education is the key when i have an anxious patient. Most of the time, it is the unknown that scares us. I go through the procedures, risks, benefits and complications thoroughly. The consultation normally lasts at least 30 minutes.
What can patients expect from you?

Our patients reduce their risk of future skin issues such as age spots, wrinkles, and pre-cancerous and cancerous conditions.

What trends are you seeing for hair restoration?

We are performing more and more eyebrow and transplants for female pattern baldness. Many women who have had browlifts or facelifts are concerned with hair loss where their incisions were made, and we can repair those areas. We even offer stencils and makeup pencils for patients to draw their desired shape. It helps them feel in control and at ease

About me:

Describe your philosophy.
I believe dermatology should focus on multiple therapeutic modalities to achieve a higher level of skin health.

MICRODERMABRASION

What's the difference between a chemical peel and microdermabrasion?

Microdermabrasion primarily uses crystals to exfoliate that superficial layer of the epidermis and a chemical peel uses some sort of chemical. Whether it's a TCA peel, Jessner peel or alphahydroxy, there are many peels that can chemically exfoliate the top layer of skin.

What does microdermabrasion do?

Microdermabrasion uses very fine crystals to essentially exfoliate the superficial layer of skin, leaving smoother skin. The texture just feels a lot nicer and a lot of other problems such as pigmentation and large pores can actually improve with microdermabrasion.

To keep your skin looking younger, longer, how can you keep your pores small and re-stimulate collagen?

There are different things you can do for that. Certain laser procedures such as IPL, chemical peels, microdermabrasion, even deeper chemical peels as well, such as TCA peels, can really help stimulate and minimize the pores.

Is it plausible that if you do microdermabrasion and chemical peels in your 20s you could prevent the need for an eventual facelift?

I don't think you could prevent the need for a facelift but you can actually prolong the need for a facelift more than anything else.

Is one of the secrets to keeping your skin looking younger, longer, having smaller pores?

It can be. If you look at someone in their 20s, traditionally their pores are a lot smaller. What happens is that over time with sun damage and the natural process of aging we lose the collagen and the fat in our skin that support our skin. So the pores are actually larger because the support isn't there to hold them together. Problems such as pigmentation and large pores can actually improve with microdermabrasion.

HAIR TRANSPLANT

Can you restore an eye brow with hair loss treatments?

Absolutely. You know lots of women either plucked their eyebrows when they were younger, some had skin cancer and there's a scar through the eyebrow, or just over time after menopause you can actually lose the outer third of the eyebrow. We can do a hair transplantation into the eyebrow to restore that. It's just amazing. We allow our patients to draw what type of arch they want and where they want it placed and they are ecstatic. You can do what you want with the new eyebrow because it's your own hair.

CHEMICAL PEELS

Where do injectables and fillers fit in with chemical peels? Does that combination just refresh a layer of skin or provide qualities people look for in Juvéderm, Restylane, Thermage and Ulthera?

Fractional resurfacing does help stimulate collagen production. It's never going to replace fillers or facelifts because they work at all different layers of your skin. So where the fillers are injected the lasers don't go that deep, they are a little more superficial so we always tell people that generally a combination of therapies actually make you look more natural than if you just did one or the other. Because the fillers and fat transfer go much deeper than what a laser can do, they build the framework from that level within the skin. When you're doing a laser procedure you're looking at tightness of the superficial portion of the skin and sun damage and fine lines.

Are anti-aging results best achieved by removing wrinkles, tightening skin or a combination?

Definitely a combination. In order for you to look natural and refreshed I do not recommend completely removing your wrinkles because that looks unnatural. So when you smile you're supposed to have some crow's-feet. You're supposed to have laugh lines. You're supposed to be able to move some muscle when you have Botox, you're not supposed to look frozen and wrinkle-less. It really is a combination of softening the face, the wrinkles, stimulating the collagen with topical antioxidants, topical retinoids and overall improving the health of your skin.

How can you treat sun damage on a chest? Are lasers or chemical peels the way to address them?

Generally we do not do deep chemical peels on the neck and chest because you scar badly from them on the neck and chest. But what we do tend to do nowadays is the newer fractional laser technology which can go down to the neck and chest. So if it's an issue of sun damage and brown spots then we use IPL but if it's from the wrinkling and in the cleavage area we use fractional laser treatment. It won't take it away completely but it will improve it.

My skin has so much sun damage and lots of brown spots. I’d like to do something on the surface only. What kind of peel goes deepest and is most refreshing?

For brown spots and sun damage there are a lot of options available. You can do something conservative such as topical lightening cream, as well as a topical retinol. If you want to be a little bit more aggressive you can do an IPL series or a photo facial series. IPL works very well for people who don't have any downtime or have to go back to work, and there are rarely any complications. If you want to get a little bit more aggressive and there are other problems such as large pores or fine lines, thinner skin, you can also do an ablative resurfacing procedure which tends to be geared more toward people with many other problems including large pores and fine lines. Chemical peels are also a very good option. If you don't have down time you can do some of the milder peels or a glycolic one. If you want to be more aggressive and you do have down time you can do the stronger TCA peel.

Is a laser better than a chemical peel?

The ablative laser is even better than a TCA peel. For TCA you're taking the top layer of the skin off. Essentially you're making the skin peel with essentially a fresh new layer which then subsequently makes it look very smooth and refreshed and the texture is great, you also get a little bit of tightening from it. The fractional resurfacing procedures these days are laser procedures and those are great as well because the downtime is about seven days of redness with crusty scabbing and for seven days you also have a pink color. And then over the course of six months the texture, the tone and the pores improve significantly.

Is it plausible that if you do microdermabrasion and chemical peels in your 20s you could prevent the need for an eventual facelift?

I don't think you could prevent the need for a facelift but you can actually prolong the need for a facelift more than anything else.

VEIN TREATMENTS

Which laser treatment can get rid of broken blood vessels on my face? There are some tiny vessels near my eyes and some bigger ones on my cheeks. Are these removable with the same laser or do I need multiple appointments?

You will probably need multiple appointments and there are actually many different lasers that can help with that. IPL can be a very useful laser. It actually picks up a couple of different things, redness on the face, as well as brown age spots. A yag laser is very helpful for individual veins and there's a pulse dye laser that is very helpful for overall redness and broken capillaries.

What's a nonsurgical treatment for the neck?

If you have a lot of redness and sun damage on the neck you can do laser treatments like IPL, if it's just a little bit more of the texture. Sometimes as we get older our necks have that texture of chicken skin that's very rough and so a laser resurfacing procedure can help with that also.

How can I remove brown spots on the back of my hands, arms and legs?

On the back of hands there are many laser procedures you can do to remove brown spots. You can do IPL. Nowadays I'm doing fractional resurfacing on arms and hands and it takes about three treatments and helps to lighten the brown spots. It doesn't take them away completely but they aren't as dark. There are health and cosmetic reasons to remove the spots. It's important to clear up the skin on the hands because a lot of times if you work on the face and the hands don't match it looks horrible because your hands also show your age, as do the neck and chest. So a lot of times when I do a consultation for the face, especially a laser consult, I'll also check the neck and chest to make sure it's not something I have to treat as well. Otherwise, if you take a photo in a v-cut shirt, your chest and neck will show.

FACELIFT

Does a skin tightener deliver the same results as a facelift?

With a procedure like Thermage you can get some tightening of the underlying tissue. With the laser resurfacing you can also get some tightening of the tissue but if there's enough in laxity then you probably truly need a facelift. Lasers like Thermage never replace a facelift in the event that you actually need a facelift. They can be used preventatively though.

I want to relax the wrinkles on my face and make my skin look tighter, should I use a filler like Juvéderm or a relaxer like Botox? Are there other brands or things I should consider?

A relaxer like Botox actually works quite well to relax the muscles that create most of the wrinkles. The other things that can actually help are chemical peels and microdermabrasion. Laser treatments can help smooth out the wrinkles and improve the texture and the tone of the skin.
