Full Face Makeover

Dr. Menachof believes that the best way to achieve complete anti-aging results is to combine both invasive and noninvasive procedures. To this end, he created Aesthetics Medical Day Spa to integrate with his surgical practice. This allows patients to receive treatments and products that complement invasive work. Surgical procedures involve pulling and stretching, and we often forget about the quality of skin on top, Dr. Menachof says. It is important that we take care of ourselves fully so that we can achieve more comprehensive results. For example, patients who seek one of Dr. Menachof's most-requested procedures, the facelift, have the option of fillers such as Juvederm and Restylane for fullness and volume. Dr. Menachof believes services such as microdermabrasion, lasers and medical-grade skin-care products that treat the skin are extremely important. These treatments not only make the skin look refreshed and healthy, but they actually improve its quality, too.

Revitalizing Facelift

Concerned with her aging appearance that caused volume loss in her cheeks and sagging skin on her jowls, this 62-year-old patient consulted with Dr. Menachof. What procedures do you perform the most?

I most frequently perform facelifts, browlifts, eyelid surgery and rhinoplasties.

What nonsurgical procedures do you offer?

I created aesthetics medical day spa to complement our surgical practice. Here we offer a variety of skin-care treatments, including lasers, microdermabrasion and chemical peels.

What is your chairside style?

It is important that my patients are comfortable and assured that they have put their trust in the right person.

What is unique about your treatment style?

My treatment style is very individualized. Three people may ask for the same procedure, but it will be different for each because everyone is unique.

How do you ensure patient satisfaction?

I spend a great deal of time consulting with my patients about their procedures. My 20 years of experience, combined with our imaging software, allows us to achieve the results our patients desire.

How would your patients characterize you?

My patients would describe me as knowledgeable, ethical and committed.

Why did you become a facial plastic surgeon?

I loved that facial plastic surgery combined anatomy and physiology, which are intricate and complicated, with an artistic aspect. Every patient and surgery is different, and I enjoy the challenge.

Do you have any personal interests outside of medicine?
I really like photography, snowboarding and spending time with my wife and kids. I also enjoy fly fishing and cooking. I might have chosen to be a chef had i not found my passion in medicine.
In what ways are you philanthropic?
I am a supporter of many Colorado charities including Adam's camp, the American Heart Association, Doctors Care, the Kendall and Taylor Atkinson Foundation, and the Colorado Neurological Institute. I believe it is important to give back.
What is your favorite part of your job?

I enjoy the interaction with the patients, when they return smiling because they look and feel so great. That is the most rewarding.

LIP ENHANCEMENT

What is the difference in recovery from lip implants versus lip fillers?

I use both lip implants and lip fillers. The recovery for a lip implant is about a week, but within five days most of the swelling has gone down. For filler treatment, if you’re lucky, the lips could look good the next day. However, there is always potential for bruising with fillers.

What is a lip lift? And what does it treat?

A lip lift is for a problem that is difficult to treat. It’s for a patient who has really thin lips. If we try and add volume, there is so little natural lip there, that the lips will end up looking like a duck’s. What you want to do in that situation is to lift the lip upwards so you see more of the pink part of the lips. Then you can add filler to the pink part and sculpt the lip shape.

CHEEK AUGMENTATION

When does a cheek implant make more sense than fillers?

It’s a personal preference. The nice thing about cheek implants is that they are forever and there are many different sizes and styles to help customize them. The cheek implants sit on top of the bone so you shouldn’t ever see them because they are so far under the skin. They are a little bit better hidden than other implants. The new fillers have changed preferences a bit. I use a lot of Radiesse now in the cheeks and it lasts quite a while. Radiesse goes all the way down to the bone so it acts much like a cheek implant. I’ve done a lot of cheek implants in the past and I don’t do hardly any today.

Can a filler change the shape of your eyes or is an eyelift or browlift procedure the only way to do that?

Fillers can do a pretty good job of it. Then if you add a little filler just underneath the brow [off label] it causes the light to highlight the brow bone, which gives the appearance of a more youthful brow. For the lower lids, you can add fillers into the groove [off label] under the eyes to get rid of that tired look.

FACELIFT

Are there alternatives to a facelift?

As we age, the muscles and tissue overlying the muscle and the cheeks starts to come down and forward. So you get heaviness towards the bottom of the face and jowling. A youthful face has most of the volume in the middle part of the face and more of an oval shape. As we get older, that oval becomes a square. Now a facelift actually takes the muscles and tissues and lifts them back where they belong. A filler is not going to do that, but it can take some of the heaviness away from the bottom of the face and restore that more youthful, full oval shape.

What is a less invasive facelift option?

I’ve tried a variety of different options and now I just offer two types: the traditional facelift and my less invasive facelift is variation of the MACS lift and lasts about 8 to 10 years. This is probably the least invasive of the facelift options that still lasts a long time.

Is a cheeklift the same as a mid-facelift?

This is all semantics. I think if you’re asking for either one of those things, you’re talking about treating the same area. Many times when people come in and say they need a necklift or a cheeklift, they really need a facelift because it’s all part of the same process. If you do a cheeklift and the jowls come up, you may notice that the jawline isn’t as crisp as it should be. In women, isolating areas often doesn’t make sense.

INJECTABLES AND FILLERS

Can Botox or Dysport help shape my brows?

Yes. At my practice we use a little Botox in the tail end of the brow [off label] to get a little brow elevation.

Can fillers like Juvéderm and Restylane be received at the same time as Botox and Dysport treatments?

Yes. There are very few situations where you wouldn’t be able to use both at the same time. They are going into different places. The Botox or Dysport is going into the muscle and the filler is going just under the skin or deep to the muscle.

Are permanent fillers a good idea?

I don’t use permanent fillers at my practice, and I don’t think they’re a good idea for a couple of reasons. 1) If it doesn’t turn out the way you want, you’re kind of stuck with it. 2) The face changes over time and what looks good when you’re 55 might not still look good when you’re 70.
