Unhappy with her short-looking teeth, this 25-year-old patient sought advice. "We performed gum contouring to even out the gum line and lengthen teeth and then placed 10 upper, minimal prep G. Lewis veneers to fill in gaps," Dr. Lewis says. "The G. Lewis veneers helped create a more beautiful smile without the need for temporary restorations."

What is the difference between a home teeth whitening kit and one provided by a dentist?

The ingredients of the whiteners you get at the dentist office are the highest concentrations of the active ingredients, which is more efficient in changing the color of the teeth. They work better and will give you a better result, but I will say that over-the-counter whitening products do work. They just take a lot longer. Crest Whitestrips for example work; however, they don’t cover all of the teeth, which is one drawback. On the other hand, some patients come in because they didn’t get the result they wanted from over-the-counter whiteners. What’s good about going to the office is that the dentist can monitor the treatment and make sure it’s done safely.

SMILE MAKEOVER

How much can tooth contouring change my smile?

You can get a dramatic change from tooth contouring depending on the situation. If you have pointy teeth and you don’t want that or if you have uneven teeth, it’s amazing what tooth contouring can do for your smile.

Can a gum lift erase my gummy smile and how much will it hurt?

I wouldn’t say it’s painless. It can be uncomfortable but usually it’s more of a tenderness for a few days to a week that can be managed with Advil. I would say it has mild to moderate discomfort. It can give dramatic results.

Can a smile makeover help me look younger?

When we’re doing veneers and full mouth reconstruction, we’re often doing something called opening the bite where we restore it to how it was before they ground their teeth down and lost the vertical height in the lower third of the face. I call it a smile lift. What were doing is building the teeth up to support the face for a more youthful look.

With a porcelain crown, does the color stay forever?

The porcelain itself is not going to change colors, but it’s kind of like eating on fine china. If you eat blueberry pie on fine china, the plate is going to look stained but if you wash it off, then the stain will go away. So when people notice crowns or veneers getting yellower, it really just stains building up on the surface. If they have them professionally polished, then they can restore the color and luster. However, the tooth underneath can get darker with age and that can show through the thin veneers. It’s not something we see a lot, but we have seen it.

What’s the best way to get a natural-looking smile?

You can get a natural-looking smile through many different treatments. Veneers can look very natural, but going too white may make them look less natural. I find that a patient’s preference is sometimes a matter of geography. My patients in Texas tend to want their teeth whiter than people do in other parts of the country. However, whiter teeth do make you look younger so you have to find the shade that fits your goals.

What are the advantages of no-prep veneers?

The obvious advantage is that you don’t have to remove any of the enamel, so your teeth are not altered. Some people like the security of knowing that if something were to happen in the future, then they could have them taken off and it would be like they had never done anything to their teeth. It’s more conservative, which patients like. I developed my own type of no-prep veneers called G. Lewis Veneers, and I did that because there are others on the market that I didn’t feel looked very good. So I developed these to be more lifelike and precise. I was concerned about them being too bulky, but when I committed to creating G. Lewis Veneers, I was surprised at how thin we could make them.

When do you need crowns instead of veneers?

If someone has a large filling or broken down tooth, sometimes a crown is a better option. Or if it’s way out of alignment and the upper and lower teeth don’t touch, you can use a crown to help fix the bite. If the teeth have had root canals, the teeth might need to be crowned because they tend to be more brittle, but not all the time. For the back teeth, I would definitely recommend a crown if they have had root canals.
