Breast Matters

Dr. Greene counts breast augmentations among his most popular procedures, with well over 1,500 breast surgeries to his credit. “We are meticulous and consult with our patients for as long as necessary to deliver results that complement their body type, lifestyle, and desired look,” says Dr. Greene. Both saline and silicone implants are available, and the choice of implant depends on patient preference and Dr. Greene’s recommendations. In addition to augmentations, Dr. Greene performs breast reductions and breast lifts, which can also be combined with augmentation when a patient seeks multifaceted rejuvenation of the breasts. “I am excited about the next generation of breast implants,” says Dr. Greene. “As plastic surgery evolves, I am able to offer patients more innovative options, which translate into natural-looking results and greater patient satisfaction. This in turn makes me very proud to be a plastic surgeon.”

Aesthetic Elevation

Dissatisfied with the lack of volume and droopiness of her breasts, the 35-year-old patient shown here consulted with Dr. Greene. “In addition to enhancing her breasts with 275-cc silicone gel implants placed subpectorally, I also gave her a breast lift to eliminate sagging,” Dr. Greene says. “She loves her new, fuller shape.”

Philosophy

Dr. Greene’s ability to listen carefully and understand his patients’ goals helps him provide a unique and personal approach to each patient. “Our goal is to put our patients at ease, listen carefully to their goals for surgery, and provide them with all the information and tools they need to make an informed decision about cosmetic surgery.” Board certified in both plastic surgery, and oral and maxillofacial surgery, Dr. Greene performs procedures for the body and face—including breast augmentation and lift, tummy tuck, liposuction and rhinoplasty— and offers aesthetician- and physician-administered in-office aesthetic procedures. Many times Dr. Greene will combine a variety of procedures in his surgical plans, custom-tailoring the procedures to the individual patient. “Combining procedures such as a breast augmentation or a breast lift with a tummy tuck via a “mommy makeover” can give women renewed confidence,” he says. Safety and comfort for his patients are key concerns of Dr. Greene, who offers patients the option of having a nurse accompany them home to make sure they are well taken care of. “The best advice I’ve received was to always respect my patients, listen to them attentively, and remain humble enough to be their partner,” says Dr. Greene, who encourages prospective patients to visit his web site and view the online videos, to have a better understanding of what various procedures involve.

Dr. Greene takes as long as needed during the patient’s in-depth consultation to make sure each question and concern is addressed. “Cosmetic surgery is elective, so choosing a plastic surgery practice that provides comfort, safety and personalized care from a dedicated staff helps ensure quality results and a positive experience.”

Targeted Tummy Makeover

Unhappy with the loose skin and fullness in her abdomen and hips after having two children, this 32-year-old patient turned to Dr. Greene for a midsection mommy makeover. “By combining a tummy tuck with contouring and tumescent liposuction of the hips, we were able to redefine her waistline for a dramatically different figure,” Dr. Greene says.

A Diverse Medical Background

With a diverse medical background spanning the breast, body and face, Dr. Mark W. Greene brings a meticulous eye for detail and a warm, comforting manner to his San Antonio, Texas, practice. “Every patient, every body, every face is unique and I approach each patient with that in mind.”

About my practice:

What is a consultation with you like?

We know it takes a lot of courage to step across that threshold into the office, so we try to make the patient feel as comfortable as possible. We first meet in a well-appointed, softly lit room to go over goals, health history, and the examination process.

What are the benefits of combination procedures?
I very often combine a variety of procedures in my surgical plans, custom-tailoring the procedures to the individual patient. Combining procedures such as a breast augmentation or a breast lift with a tummy tuck via a 'mommy makeover' can give women renewed confidence.
What are your top priorities when treating patients?

Safety and comfort are key concerns, along with striving for an exceptional result.

What should patients keep in mind when choosing a surgeon?
Cosmetic surgery is elective, so choosing a plastic surgery practice that provides comfort, safety, and personalized care from a dedicated staff helps ensure quality results and a positive experience.
What is your operating room style?
I treat each patient the way I would treat a member of my family.
How do you educate patients?

In addition to helping patients feel comfortable, we make sure they are well informed about their procedure, the surgical process, and recovery. I always think how I would like to be treated if I were the patient, so I spend as much time as we need to answer any questions and discuss their procedure in detail.

How does your staff contribute to the patient experience?

The key to our success and our patients' happiness is not only the execution of the surgery, but the artful care and attention to detail they receive from our staff. They are so competent, personable, and caring. Patients sense that the minute they walk in the door.v

About me:

What is the best advice you've received?
I was taught to always respect my patients, listen to them attentively, and remain humble enough to be their partner.
Why did you become a plastic surgeon?
It is a fascinating specialty of problem solving.
How would you like to be remembered?
I'd like to be remembered as someone who cared for others.
What are your hobbies?
I have a passion for the outdoors, including skiing, hiking, and scuba diving.
Have you done any pro bono work?

Yes. I traveled with a team to Puebla, Mexico to treat cleft lips and palates. I had a good friend who initiated the program and I was fascinated by it. The chance to help those in need was so rewarding.

What is your favorite part of your job?
Interacting with my patients and knowing I was able to make a difference in their lives is very rewarding.

NOSE JOB

I’m having a hard time breathing through my nose, do I need a nose job?

Rhinoplasty is typically an aesthetic procedure. There are other procedures called a septoplasty and a turbinectomy that can be done to open up the airways. Sometimes because of trauma or developmental deformities, there can be some curvature and deviation in the septum where one side is more open and the other is more closed. This patient would benefit from seeing a plastic surgeon who is very familiar with the internal structure of the nose or an ear nose and throat doctor. Now, if you’re having a breathing issue and you want to enhance the shape of your nose, it’s simple to combine the procedures so there is only one recovery.

How long is the recovery from a nose job?

If you make cuts inside the nasal bone to narrow the shape, that causes some bruising and bleeding and that’s why people have black eyes after rhinoplasty. This typically goes away in a couple of weeks. If that is not done, there is very minimal bruising and the healing goes faster. The worst thing about a rhinoplasty is that the nasal tip will be a little swollen for about six months. It’s not swollen in a way that other people will notice, but the patient will notice and it takes a long time to go down.

BREAST LIFT

How long does a breast lift last?

It depends on several things: genetics, quality of the tissue and whether or not the patient has a healthy lifestyle. If an implant is placed, the size of the implant will also affect the lift. The age the patient underwent the breast lift is also a factor so it’s hard to generalize.

Can a breast augmentation restore symmetry?

It depends on the asymmetry. If there is tremendous asymmetry, such as the left breast being a size B cup and the right breast being a size D cup, that’s almost impossible to treat with an augmentation alone. In that case, I recommend a two-step procedure to reduce one breast to the size of the first and then augment both breasts. On the other hand, if you have someone with a mild volume difference and there is no difference in the overall shape of the breasts and height of the nipples, then a lot of times we can choose one breast to be the standard, augment that breast and then use a sizer on the other side, place a temporary saline implant and adjust it until we know the exact amount of ccs we need to match the first breast.

How can I determine which breast implant is right for me?

There are two basic types of implants: saline implants and silicone implants. The silicone implants have been back on the market for five years, and, in my opinion, have a more natural look and feel than saline implants. They are also less likely to rupture. The next big question is about size, which can be determined via a thorough consultation. We let patients try on the implants in a bra in the office to get an idea of what that proportion will look like. Another issue is textured versus smooth implants and teardrop versus round implants. Most people choose round and smooth implants because there is a smaller chance of wrinkling or rippling and as they move in the pocket, they will appear to have a more natural shape. Textured implants don’t give as much movement.

BREAST AUGMENTATION

Are breast implants affected by pregnancy?

Not the implants themselves; however the breast tissue will act exactly the same as it would without an implant. For example, if the breast was going to enlarge significantly, it will still do that. Some patients are lucky and even after pregnancy the skin contracts back over the implant nicely. Other patients have thinner skin and develop excess skin and stretch marks and might consider a breast lift after their childbearing years.

BREAST IMPLANTS

Will my breast implants eventually have to be replaced?

The manufacturers tell us to inform our patients that these are not lifetime devices and that sometime in the patient’s life the implant will have to be replaced. On the other hand, the American Society of Plastic Surgery states that if there is no problem with the implant, then they wouldn’t recommend replacing it. The implants from the 70s and 80s had a lifespan of about 15 years, but those were much different implants than what we use today. We expect today’s implants to have a longer lifespan.

TUMMY TUCK

What is the difference between a mini-tummy tuck and a traditional tummy tuck?

If you just have a little bit of skin to remove, we can do a mini-tummy tuck. However, if you need to remove a lot of skin and fat and stretch marks, then you need an abdominoplasty or traditional tummy tuck. We also tighten the fascia so we have a firm foundation upon which to remove the skin and fat and bring the layers together for a nice post-op abdomen.

FACELIFT

Where are the typical scars for a facelift?

The traditional facelift scar is in the temporal area just above the ear and it comes down along the curvature of the ear. My incision goes inside the ear and then around the earlobe and back of the ear and will dovetail into the hairline or will parallel the hairline. There is also one under the chin in the natural crease and that allows us to take care of muscles in the neck and to remove any fat in that area.

Is there preventative anti-aging work I can have done before I would need a facelift?

Yes. Three important things: don’t smoke, use a good sunblock and use retin A. These things will really help you with aging of the face. Smoking especially ages the skin and adds lines around the mouth. Sunblock will help prevent the damage that occurs as a result of sun exposure and then retin A will build collagen in the skin and exfoliate the outer layer of skin to improve the overall health of the face.
