Rejuvenating Without a Facelift

This 68-year-old female patient came to Dr. Callahan with concerns of volume loss, skin laxity and wrinkles on her face. Dr. Callahan chose to use Botox on the patient’s glabella, forehead and crow’s-feet, and Sculptra for lateral volumization and to refresh the temple area. “The patient is thrilled with the results and feels years younger,” says Dr. Callahan.

When evaluating the skin and rejuvenation goals, Dr. Callahan looks at three key areas: skeletal structure, facial volume and the condition of the skin. “This particular patient had severe photodamage as well as volume loss,” Dr. Callahan says. “So it was essential to combine volume with skin resurfacing to achieve the best possible result.” After a Fraxel re:pair treatment, a series of dermaplaning and peels, as well as treatment with Juvéderm and Botox, the patient appears younger.

“A single modality isn’t going to correct all of your concerns,” says Dr. Callahan and continues, “many patients require a combination of lasers, fillers, Botox and cosmeceuticals.” She offers a suite of premier lasers and light-based sources—including IPL for brown spots and rosacea; Vbeam laser therapy for facial redness and scarring; fractionated resurfacing for wrinkles and acne scars; Pellevé for tissue tightening; and a cosmetic “toolbox” of fillers for volume restoration. “The more modalities you have at your disposal, the more targeted the treatments,” says Dr. Callahan, who, when analyzing the skin, breaks it down into three different layers. “We’ll first start with the epidermis, considering tone and texture. Then we progress to the next layer, addressing volume loss. Then we’ll move onto the deepest layer, tightening underlying tissues when necessary.”

How would you describe your chairside style?

It's educational, straight-forward, collaborative and thorough.

What is your vision for cosmetic dermatology in 2012?

Creating volume in the mid to lateral upper face will be at the forefront in 2012.

What do you see when you study a patient's face?

Many patients are already “well volumized” in the peri-oral area— in the lips and smile lines—through the use of traditional fillers. But they need help in the lateral areas—the temples, the cheeks and the jawline. I complete the picture by connecting existing volume in the peri-oral with new volume in the lateral areas.

How would you describe the result?

When balanced volume is achieved, it is difficult for the natural eye to detect what I have done. People know that a person looks better and refreshed but they can’t figure out why. It’s a totally natural look— and a complete treatment!

In what ways is your approach more than skin deep?

I employ a results-oriented philosophy that incorporates a clinical, pathologic and cosmetic background. My experience as a dermatologic surgeon translates into what I call a Renaissance approach—one that treats common concerns like texture irregularities, volume loss, pigmentation and scarring.

How are you conscious of patient empowerment?

I spend a great deal of time educating patients. Such a patient-guided methodology enables me to customize a treatment plan in response to each patient’s needs and downtime allowances. We walk patients through procedures and call all first-time cosmetic patients following treatment.

How far does your clientele reach?

My patients come from the entire west coast of Florida—from Tampa to Naples.

What was your experience leading up to your current private practice?

To really sharpen my skills, I advanced my training at two respected institutions. In particular, I performed my residency and a dermatology internship at the Cleveland Clinic Foundation in Cleveland, Ohio, and completed a fellowship in dermatologic and Mohs surgery at the Mayo Clinic in Rochester, Minnesota.

What personal trait has benefited your practice?
My curiosity. This helps me provide the latest treatments and newest lasers from around the world.
What can I do to correct the hollows around my mouth?
Restylane, Radiesse, Juve_derm, Perlane and Sculptra are all indicated for the parentheses around your mouth, plumping out your features for a more robust, youthful appearance. Best of all, the results are immediate and the procedure is nonsurgical, taking just a few minutes. While there's no one filler that's best for everyone, I can gladly assess your skin to determine what dermal filler will best bring about your desired aesthetic goal.

The best candidate is someone with dark hair and fair skin.  Depending on the treatment area, you may need 5 to 10 treatments. It can be used almost anywhere on the body; however, it does not treat blond or grey hair.  Lasers used for laser hair removal include the Alexandrite, Diode or Nd:YAG.

Is CoolSculpting by Zeltiq painful?

The process can be painful when first applied; it then becomes numb and less painful during the remainder of the treatment. Afterwards the treated area may be a little tender to the touch for a few weeks. For the most part, there is minimal or no downtime and you can resume most normal activities the day of treatment.

Usually a combination of treatments is best; including Intense Pulsed Light, lasers, chemical peels and on-going at-home topical treatments recommended by your dermatologist. Of course, the best way to prevent brown spots long-term is with sunscreen.  I recommend using one that has SPF 30 or higher, contains zinc or titanium above seven percent (I prefer a micronized formulation).  Also, it must be broad spectrum to block both UVA & UVB rays.

Where is Botox used?

Botox is used in a number of locations (some areas off label). In my office, most commonly between the eyes, the forehead and brows, crow’s feet, the lips, the corners of the mouth and the neck. It is also used for hyperhidrosis (excessive sweating). If used regularly every four months, the results tend to get better and better over time and fewer products may be needed.

What makes Sculptra different from other fillers?

Sculptra works as a collagen stimulator. Its activity works best after 3 separate injections (six weeks apart) and can last up to two years.
