Although still one of the most popular aesthetic surgery requests, breast augmentation cannot fix every breast-related concern. Especially if both location and volume of the breasts are issues, Dr. Benedetti cautions patients to do their homework and get several opinions before choosing a plastic surgeon. The easiest solution isn’t always the best one, and he has seen countless revision cases that could have been prevented. “In many cases, instead of putting the implant where it belongs and lifting the breast, the surgeon will place an overly large implant too low to try to restore the volume. The problem with that is that it leads to a worsening breast droop in a short period of time,” Dr. Benedetti says. “The first time is the best time to get it right. People are worried about doing a more extensive procedure [mastopexy and augmentation] but, in some cases, it’s better to do that than to do a revision surgery.”

When looking to enhance breast volume, potential patients don’t always realize how many decisions are involved aside from size selection. “Generally, the easiest part of the process is picking a size, which is what we do at the pre-op visit,” Dr. Benedetti says. “I actually don’t do sizing with them the first visit because I really want to talk to them about the choices they have including incision location, implant placement and the type of implant. These are important decisions as they can affect the results long-term. For example, implants placed behind the muscle can help reduce the chance of long-term issues related to scar tissue formation, capsular contracture, or the chance of implants bottoming out or lowering with time.” Acting as a guide throughout the process, Dr. Benedetti helps narrow down the best choices based on the individual patient’s aesthetic goals, measurements and lifestyle. “It’s always time well spent to listen carefully to what the patient wants, even if it takes multiple consultations,” he says.
Facelifts Should Go More Than Skin Deep
When educating patients about their facelift options, Dr. Benedetti first points out that there is no such thing as an easy facelift that gives quality results. “To get a long-lasting result, there isn’t a quick fix,” he says. This is why he spends time explaining the difference between less complete procedures and those that make more significant improvements, such as the deep plane facelift. “I educate patients on how I lift the SMAS layer of tissue internally and, in addition, lift the skin on top in a natural direction. This is very different than the minimally invasive lifts that don’t touch the internal structure, address only the skin and frequently leave patients with less overall correction.” Clinically we see that if you lift the deep structures and maintain the support on them, you get a better and more natural-looking lift with more longevity,” Dr. Benedetti says. Since his patients want long-term correction, he is happy to tell them they can expect the results to last from 10 to 12 years on average.
