Dr. Terry L. Donat believes in merging his patients’ aesthetic interests with their recognition of youth. As a facial plastic surgeon, his philosophy is a substantive reflection of these two powerful forces. “A face is the source of all expression. It is your uniqueness on display,” he says. “Nevertheless, in time, you may come to feel that your facial appearance no longer reflects your inner character.”
Recognizing that undergoing facial plastic surgery is a highly personal decision, Dr. Donat is careful to read between the lines when consulting with patients, so that he may truly understand the features that they most highly value. Only then does he offer alternatives. “My current efforts in recreating a more youthful, natural facial appearance center on ensuring that the volume and contours of soft tissues in the temples, cheeks and lips—all indisputable characteristics of a youthful face—are fully preserved or proportionally reinstated.”
At The Institute of Aesthetic Surgery (IAS), located in suburban Chicago, Dr. Donat specializes in both surgical and nonsurgical facial procedures. He is one of fewer than 20 surgeons board certified in facial plastic surgery in the State of Illinois.

RHINOPLASTY: FORM AND APPEARANCE MATTERS
“If I had to do only one procedure for the rest of my life, this would be the one,” confides Dr. Donat about RHINOPLASTY. Because of the complexity, variety and the fact that the nose is the central feature of facial identity, he finds it a most compelling, satisfying surgery. “The form on the outside does not necessarily portray what is on the inside. We are not working with alabaster, clay or wood. There is unpredictability with tissues that must be anticipated, both structurally and aesthetically, for the best outcome.” According to Dr. Donat, the most challenging noses are those with skin that is either too thick or too thin. “With thicker skin even a DRAMATIC CHANGE may not be as apparent because the skin acts as a cloak, obscuring the new result. We try to be as aggressive as possible to reduce volume, but thicker skin on the nose poses the greatest aesthetic visual challenge for any rhinoplasty surgeon.” Thin skin provides the opposite pitfall. As it heals, every irregularity, pigment change or bump may become apparent. “I will often use ALLODERM®, a well-accepted soft tissue graft, and lay it directly underneath thin skin. Its placement over the bone softens the contour of the nose more effectively than the patient’s own skin.”
