Carbon dioxide dominates wrinkle fighters

Posted Thursday, July 22, 2010

A recent study by the University of Michigan substantiated the opinion of those who believe carbon dioxide laser resurfacing treatments are the gold standard for wrinkle removal.

According to the researchers, this kind of procedure is more effective than some newer wrinkle treatments due to cells' collagen-producing response to the damaging vaporization of water molecules.

"Use of the laser allows precise treatment, giving the surgeon more control over the resurfacing procedure than is possible with other techniques such as chemical peels and dermabrasion," Dr. Daniel Ward and Dr. Shan Baker wrote in the Archives of Facial Plastic Surgery.

While the results are very pleasing, the study participants weren't free from side effects, which included discoloration, herpes and cold sore outbreaks, and acne or milia eruptions. Researchers report that all were resolved with treatment.

In my opinion, laser resurfacing treatments can provide quick “fixes,” but the effects aren’t long-lasting. Yes the wrinkles appear less deep and the surface is smoother, but only for a short time. When a client asks for a “strong” peel, saying, “I want my skin to look like a baby’s,” I have to explain that that’s not how things work. There is a big price to pay for this constant eroding of the outer layer of the skin. As I’ve said before, you can lift and tuck, but you cannot make the skin thicker. Since our skin thins as we age anyway, we really have to ask ourselves: Just how much resurfacing can we afford to do? And that only addresses the long-term issue of erosion. The article also mentions several immediate side effects, including discoloration, herpes and cold sores, acne or milia eruptions. Wow! That’s quite a list. First, let’s look at the pigment (discoloration) issue. This alone is a huge problem, because once you disturb pigment cells (melanocytes), the condition will be with you forever. You will need ongoing treatments. In my experience, issues with pigmentation are the most difficult to keep at bay, so I am reluctant to use “treatments” that cause such problems. The same goes for cold sores. They, too, can be persistent and difficult to control. I definitely think laser has a place, but, again, only in moderation. And it is extremely important to explain the pros and the cons of the treatments. As skin care professionals, it is important for ALL of us to educate our clients/patients so they will receive the greatest benefits from any procedures and also at the least “cost.” Fortunately, we are living longer, but that also means that we must think more into the future when it comes to our skin care. Immediate can be good, but long-term is the real key.
